Home Health

When you or your loved one are no longer hospitalized, yet unable to leave home for treatment or therapy without risk, the registered and licensed practical nurses from Sierra Home Health, under a physician's guidance, will provide skilled nursing care in the comfort of your own home. We'll coordinate with physical, occupational, or speech therapists (who come to your home as well), and maintain communication with your physician as needed.

Examples include (but are not limited to):

• Patient evaluation & observation
• Development and implementation of a care plan
• Medication management and administration
• Wound care & dressing change
• Pain management
• Diabetes care
• Venipuncture & IV therapy
• Catheter care
• Patient & family emergency & in-home safety education
• Enteral, parenteral & standard nutrition
• Tracheostomy & gastrostomy care
